The job role is a full-time position in a busy and friendly Older Adult Mental Health Team that work within a well established integrated service. The successful candidate will work with a band 6 nurse to deliver Initial assessments for individuals who have been referred with a deterioration in their cognition. The Service provides assessment, support, education to service users, their families and carers. This can be a fast-paced role and requires good organisational skills and excellent communication skills. You will be motivated and an effective team player. Main duties of the job Support Band 6 Nurse in the delivery of initial assessment and reviews. Deliver Cognitive assessments at initial assessment and reviews. Support Initiation and compliance of Anti-dementia medications under the supervision of registered nurses. Telephone support and contacts to service users and their families and carers. Liaising with third sector agencies and support services. Participate in MDT and professional meetings. Co-working with the local authority. Making referrals for identified needs. Build relationships with families and carers. To report identified risks to team and medic. Familiarise and have an understanding of the demographic of the local area. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
