Saturday (midnight to midnight) and any week day after 8pm and before 6am.
Job responsibilities

  • Support the Senior Practitioner in the implementation of processes and systems to facilitate safe and effective discharge for service users in a timely manner.
  • Liaison with inpatient and community teams to identify barriers to discharge and reduce Length of Stay for service users.
  • Improve choice and personalisation
  • Liaison with partner organisations such as housing to encourage an integrated approach to care.
  • Proactively identify and make appropriate housing referrals, following these referrals through to completion. This will include working with people who are of No Fixed Abode
  • Resolve any other housing concerns e.g. rent arrears, applications for discretionary payments, safeguarding concerns and anti-social behaviour
  • Organising deep cleans
  • Supporting Care Co-ordinators and Patient Flow practitioners with placement searches and referrals e.g. supported accommodation, nursing, residential care
  • Supporting patients who have no resource to public funds
  • Signposting service users to community support services e.g. MIND, food banks, Welfare Rights etc
