                                                    Job responsibilities Support the Senior Practitioner in the implementation of processes and systems to facilitate safe and effective discharge for service users in a timely manner. Liaison with inpatient and community teams to identify barriers to discharge and reduce Length of Stay for service users. Improve choice and personalisation Liaison with partner organisations such as housing to encourage an integrated approach to care. Proactively identify and make appropriate housing referrals, following these referrals through to completion. This will include working with people who are of No Fixed Abode Resolve any other housing concerns e.g. rent arrears, applications for discretionary payments, safeguarding concerns and anti-social behaviour Organising deep cleans Supporting Care Co-ordinators and Patient Flow practitioners with placement searches and referrals e.g. supported accommodation, nursing, residential care Supporting patients who have no resource to public funds Signposting service users to community support services e.g. MIND, food banks, Welfare Rights etc Staff Benefits Please see attached job description and person specification Pay Enhancements: Band Saturday (midnight to midnight) and any week day after 8pm and before 6am All time on Sundays and Public Holidays (midnight to midnight) 1 Time plus 47% Time plus 94% 2 Time plus 41% Time plus 83% 3 Time plus 35% Time plus 69% 4 9 Time plus 30% Time plus 60% 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years Excellent pension Cycle to work scheme Salary sacrifice car scheme Wellbeing programme Blue Light Card Discounts fuelGenie Fuel Cards (for applicable roles)
