The Northern Care Alliance NHS Foundation Trust is seeking a part-time Community Staff Nurse (Band 5) to join the Out of Hours Team in Bury as part of the Adult Community Nursing Single Point of Access. The successful candidate will possess strong clinical skills and be adaptable, supporting the triage of referrals for District Nursing and the Specialist Palliative Care team. Responsibilities include communicating effectively with referrers, patients, and carers to ensure timely care responses and occasionally providing unscheduled care to priority patients in their homes.

This role involves working weekends and bank holidays, primarily during the day, with some twilight coverage as part of a rota.
