The Countess of Chester Hospital: providing 438 general and acute beds.
Working Full time : 37.5 per weekWe have an exciting opportunity within the Integrated Discharge Team for a Band 6 Integrated Discharge Sister.
The Integrated Discharge Team are based in Moston lodge, this post is key to facilitating complex discharges from the Acute Trust.
The successful candidate will be supported by IDT Manager and all necessary training provided. We are looking for an organised, professional and enthusiastic member of staff who is looking to further their career within The Countess of Chester Hospital.
- To work as an anonymous practitioner to perform specialist nursing assessments to patients and use clinical reasoning to deliver and adapt individualised discharge plans, involving patients and carers/relatives in the planning and delivery of their care.
- To be professionally responsible for managing a defined caseload of complex cases and lead Discharge Planning/Best Interest Meetings and Professionals meetings as required.
- To facilitate Red and Green Multidisciplinary Meetings daily on the wards to ensure active discharge planning is being undertaken to progress the patient to their date of discharge.
- To identify which discharge pathway the patient requires and in conjunction with the ward staff ensure this is documented on tele tracking at the daily meetings.
- To provide education/training to the ward staff and other members of the multidisciplinary team in all matters related to discharge planning.
- To act as a point of contact for patients/carers and others for information on the discharge process and NHS continuing health care, assisting patients to access advocacy schemes and external advice when necessary

- Ellesmere Port Hospital: providing 56 beds as a rehabilitation, intermediate and outpatient facility
- Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
The Trust is a busy district general hospital and in 2022/2023, there were more than 503,342 patient attendances (inpatient, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery. This is an increase of nearly 10,000 compared to the previous year when there were 494,387 patient attendances.
