The London Borough of Hounslow is seeking a dedicated Social Work Assistant to join their Feltham and Great West Road Locality teams. This role involves conducting Social Care Assessments and providing Care Management support to help residents and carers identify their needs and develop tailored Care Plans. The position emphasises collaboration with a range of stakeholders, including care providers and health services, to promote safety and safeguard the well-being of vulnerable adults while adhering to the Council's commitments to equality, diversity, and inclusion.
