The London Borough of Hounslow is seeking a dedicated Social Work Assistant to join their Feltham and Great West Road Locality teams. This role involves conducting Social Care Assessments and providing Care Management support to help residents and carers identify their needs and develop tailored Care Plans. The position emphasises collaboration with a range of stakeholders, including care providers and health services, to promote safety and safeguard the well-being of vulnerable adults while adhering to the Council's commitments to equality, diversity, and inclusion.

Candidates should possess experience working with vulnerable adults and demonstrate strong communication and partnership skills to ensure effective assessments and support. The council supports career progression, offering opportunities for further training such as a Social Work apprenticeship. Flexible and remote working arrangements are also available, fostering a healthy balance between work and personal commitments while supporting the council's diverse community.
