The Health Visitor role in Pontypridd with Cwm Taf Morgannwg University Health Board is part of the Healthy Child Wales Programme, focusing on providing care for children and families from the antenatal period to age five. The Specialist Community Public Health Nurse (SCPHN - HV) will manage a defined caseload, undertaking assessments and developing personalized child plans in collaboration with families and various agencies.
