The Care Navigator role at Cheshire & Wirral Partnership NHS Foundation Trust in Birkenhead offers a unique opportunity to support young individuals and their families in the Children & Young People's Mental Health Service while they await treatment. This community-based position requires access to transport and involves direct interaction with families and young people, attending collaborative meetings, developing plans to connect them with community resources, and enhancing knowledge of local services. As part of a team that has been recognized for its compassionate and person-centered care approach, the Care Navigator will play a vital role in monitoring young people's wellbeing and raising concerns as needed.

This is an exciting opportunity to join an innovative service which supports young people and their families while they wait for their treatment. The Care Navigator Team is embedded within the Children & Young People's Mental Health Service (CYPMHS) Wirral 0-18 team who work with young people presenting with moderate to severe mental health needs.
The work is mostly community based so having access to transport is essential
