The Central and North West London NHS Foundation Trust is seeking a motivated and innovative Healthcare Navigator for their Health & Justice team at HMP/YOI Bronzefield in London. This new role involves supporting patients during their early days in custody by providing guidance, reassurance, and signposting to essential health and social care services. The Healthcare Navigator will play a vital role in identifying risk factors, promoting health and well-being, and fostering effective communication between healthcare teams and the criminal justice system to enhance patient care. Candidates should possess strong healthcare knowledge and experience in team settings, hold at least five GCSEs including Maths and English, and demonstrate excellent IT skills.
