The States of Guernsey are seeking a Staff Nurse (Band 5) for their Community Health & Social Care team in East Sussex. This role involves collaborating with Senior Team Leaders to assess, plan, implement, and evaluate care programs for Service Users living at home, managing caseloads, and providing day-to-day support and supervision to staff. The postholder will deliver high-quality social nursing care within a multi-disciplinary team, ensuring effective support for Service Users in the community.

The post holder will assist the Senior Team Leaders in assessing, planning, implementing and evaluating programmes of care for Service Users living at home within a defined caseload. The postholder will also assist the Senior Team Leaders with the management of the caseloads, day-to-day support, supervision, and deployment of staff. The post holder will provide high quality Social Nursing Care supporting Service Users in the community and will work as part of a multi-disciplinary team within an integrated Social/Nursing care model.
