To work as a supportive and inspirational clinical leader within the integrated Locality team, providing a high standard of service-specific expertise.
- Provision of leadership to the Intermediate Care team within the locality
- To be part of the locality’s management team, providing leadership to the wider, locality-based Health & Wellbeing Team
- To contribute to the strategic development of the locality in partnership with the Newton Abbot Community Services Manager and other Professional Leads
- To contribute to the strategic development of Intermediate Care within the locality in partnership with the Newton Abbot Community Service Manager/other Locality Leads
- To work as a supportive and inspirational clinical leader within the integrated Locality team, providing a high standard of service-specific expertise
- The leadership role will provide some operational oversight across South Devon Localities
- To work closely with the Locality’s GP Practices to ensure that Intermediate Care services are delivered in a safe and effective way; with the outcome of providing an alternative to hospital admission, enabling earlier hospital discharge and preventing early admission to long term care
- To develop and maintain professional working relationships with the Care Homes commissioned to provide Intermediate Care bed-based care
- To value everyone as an individual and actively support their right to have their voice heard in decisions about how their needs are met
- To value differences and individuality, showing respect to all
- To respond with humanity and kindness to every person, by listening attentively and respecting others in conversations
- To respect every person’s preferred wishes whilst providing nursing care within their own home
- To provide the highest quality of care by listening to and acting on feedback
- By taking personal responsibility you will play your part in sustainably improving services by implementing change positively and contributing to MDT discussions in a constructive and flexible way
- To work in partnership to achieve the best possible outcomes for our people
- Helping and supporting your colleagues so that you work well together to achieve common goals and objectives
- To act as a role model, valuing everyone’s opinion and experiences
- To seek out, recognise and value the contributions of others and the role they play in a patient’s life

The multi- disciplinary Intermediate Care and Community Therapies Team have a hard-working, flexible and can-do approach.
