Liverpool City Council is seeking experienced and proactive Qualified Social Workers (QSW) to join our Adult Services team. This high-impact role involves working with vulnerable adults to assess their needs, develop support plans, and ensure their well-being. Key Responsibilities
Undertake assessments of adults with complex needs, ensuring care plans are person-centered and promote independence.
Work closely with adults, their families, and partner agencies to co-create tailored support plans.
Manage cases involving safeguarding issues, mental capacity assessments, and complex care needs.
Act as an advocate for vulnerable adults, ensuring their rights are upheld and their voices heard.
Liaise with healthcare professionals, community services, and long-term care providers to coordinate services and resources.
Maintain accurate records and comply with statutory frameworks and best practices.
Apply strength-based approaches in supporting individuals to live independently and with dignity.
Requirements
Degree or equivalent in Social Work and registration with Social Work England.
Significant experience in adult social care, ideally with safeguarding and complex needs cases.
In-depth knowledge of relevant legislation (e.g., Care Act, Mental Capacity Act, Deprivation of Liberty Safeguards).
Strong assessment, risk management, and decision-making skills.
Ability to work effectively under pressure and manage a complex caseload.
Excellent communication and teamwork abilities.
