We are excited to be recruiting a nurse to work in the Discharge Lounge for our Cardiothoracic surgical wards in a world-leading cardiac centre in the heart of the City of London, who will be working alongside our team of dedicated staff, caring for patients pre and post cardiothoracic surgery. The patients being discharged via the Discharge Lounge will be those who have undergone both Cardiac and Thoracic procedures. Are you an enthusiastic and motivated registered nurse? This role has an element of autonomy so allows for forward thinking and where the successful candidate will be encouraged to develop on ideas that will benefit patient flow for all the cardiothoracic surgical wards. Successful candidates will be supported by Junior Sisters/Charge Nurse, the Ward Manager, Practice Development Nurse and Advanced Clinical Practitioners. We are proud of the fact that our nurses gain invaluable experience here at St Bartholomew’s Hospital, through which they are ideally placed to move into more senior roles, or acquire new skills. The purpose of the role is to provide a positive patient experience at the time the patient is identified as being medically fit for discharge. The post holder will work as part of a team to provide a safe, effective and efficient care environment for patients and their carers with a specific focus on safe and timely discharge from hospital. They will also be a point of contact for patients following discharge. The post holder will be responsible for facilitating the prompt discharge of 4th cardiothoracic surgical patients from the floor, through early identification of the patient being deemed medically fit for discharge and having all their discharge documentation and medications prepared in a timely manner. The post holder will provide the necessary advice and guidance to patients in relation to their safe discharge and on-going recovery. The post holder will also be responsible for contacting patients two – three days post discharge for a welfare check. The post holder will be integral to developing this role to ensure continuity and high standards of care ensuring patients are prepared pre-discharge as per the integrated care pathway and discharge care bundle. A secondary outcome of this role will be improved utilisation of bed capacity. Early discharges from the ward will enable a more efficient flow of patients from the intensive care and high dependency areas.
