Telexpress Systems Inc. is seeking a Health and Wellbeing Professional (female car drivers only) to join their dedicated Community Health and Well-being Service team in West Leeds. The role involves supporting individuals in their personalized care plans to promote independence, well-being, and community inclusion. Successful candidates will work closely with clients to identify their needs and connect them with local resources, as well as liaising with health professionals such as District Nurses and GPs.
