The Care Navigator role at PAN TRADE in Birkenhead offers the opportunity to support young people and their families awaiting mental health treatment as part of the Children & Young People's Mental Health Service (CYPMHS). Job Summary
This is an exciting opportunity to join an innovative service which supports young people and their families while they wait for their treatment. The Care Navigator Team is embedded within the Children & Young People's Mental Health Service (CYPMHS) Wirral 0-18 team who work with young people presenting with moderate to severe mental health needs.
The work is mostly community based so having access to transport is essential
Main duties of the job
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
Our aim is to help people to be the best they can be, adopting a compassionate, person-centred approach to everything we do. We were recently rated as Outstanding for Caring by the Care Quality Commission with a Good rating overall.
