Smiley & Co, Ltd. is seeking a part-time Health and Wellbeing Practitioner in East Lothian to support their Health and Wellbeing service. The role involves working alongside existing practitioners primarily within GP practices to provide vital outreach support to individuals referred from these practices. Candidates should have experience in health and social care, particularly in aiding individuals with long-term physical and mental health issues, and possess strong coaching, group facilitation, and community engagement skills.
The post holder will travel across East Lothian, managing a caseload of referred individuals and employing a strengths-based approach to help them access community resources and self-management support. Responsibilities also include co-delivering lifestyle management courses and supporting volunteers with lived experience to integrate into peer support roles within the Wellbeing team. The position offers a salary of £20,281 (pro rata) and applications are open until February 5, 2025.

The Role
Our client is looking for a part time Health and Wellbeing Outreach Worker to support the delivery of their Health and Wellbeing service in East Lothian.
The outreach worker will work alongside two Health and Wellbeing Practitioners who will be predominantly based within GP practices in Musselburgh and Prestonpans to provide outreach support to people referred from GP practices across East Lothian.

The Post Holder Will
Travel across East Lothian to engage with people referred to the Wellbeing Service
Practice persistent engagement to help people who struggle to receive support
Manage a caseload of people referred to the service
Use a strengths based personal outcomes approach to help people identify and access appropriate support for them to self-manage including:
Linking people to community resources and supports
Connecting people to the wellbeing practitioners for specific coaching in self-management support
Co deliver Lifestyle Management courses
Support volunteers/paid peer supporters who have lived experience of long-term conditions into volunteer/ peer roles to support the Wellbeing team
