The Coastal Primary Care Network is looking for an enthusiastic, self-motivated and driven Social Prescriber to join our dynamic team to work in a patient facing role. The Coastal PCN is seeking an exceptional individual to be at the forefront of this initiative.

This is an innovative role, designed to work in an outcome focused way to improve people’s quality of life, health and well-being by recognising that this can be affected by a range of social, economic and environmental factors. You will be supporting patients and their carers to achieve their personal aspirations participate in their local and wider communities, enhance effective personal support networks, enabling individuals to maintain healthy lifestyles; and lead independent and fulfilled lives. This job will on occasions involve having difficult and challenging conversations with people about their current emotional state, please consider this before applying.

You will work as part of the wellbeing team to strengthen personal resilience to improve wellbeing and reduce health inequalities. You will do this by running several clinics across the PCN, offering both face to face and telephone appointments to patients of the PCN.

You will promote patient choice by adopting a person-centred approach to your style of work and have an awareness of the wider determinants of health. In doing so, you will support people to have more control over their lives and help them to feel empowered to manage the complexities of their life. This job will on occasions involve having difficult and challenging conversations with people about their current emotional state, please consider this before applying.

In order for patients to have the best level of support, you will be a motivated, enthusiastic and driven individual with a genuine interest in helping people to thrive. In addition to this you will have understanding of the factors affecting an individuals health and wellbeing.

You will also develop a knowledge of local resource and will be the central point of contact for PCN Staff whose patients are being impacted by their wider determinants of health, offering support and advice when needed.

The Coastal Primary Care Network (PCN) is a group of 3 Practices in Teignmouth & Dawlish. We collaborate to improve services for patients by giving patients better access and by treating a broader range of health conditions. The PCN team consists of Social Prescribers, Pharmacists, Pharmacy Technicians and a Health & Well Being Coach. As a Social Prescriber, you will provide a central point of contact for patients in Primary Care, linking them with sources of support within local communities.
