Specialist Health Visitor – Caerphilly Intensive Support Team
This is an exciting opportunity for an experienced Health Visitor within ABUHB who will work alongside Caerphilly's Intensive Support Team. The post holder will plan and deliver a specialist health visiting service to families as part of the pre-birth assessment process in Caerphilly Children’s Services.
The Specialist Health Visitor will offer timely interventions, intensive support and assessments as early as 13 weeks of pregnancy. This intervention has been proven to not only reduce the number of children becoming looked after but also to reduce health inequalities.
Main duties of the job
Take responsibility for managing a caseload of individuals/families with complex needs.
Use negotiation skills to develop individual Care & Support Plans with individuals/families; continually assessing and evaluating care needs.
Analyse own practice, identify new ways of working and implement change
Undertake audit of practice, identify excellence and areas of poor performance, and be able to design programmes to meet any required needs identified.
This post is fixed term/secondment until 31st March 2026 due to funding. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.
