The Cheshire & Wirral Partnership NHS Foundation Trust is seeking a compassionate and motivated Band 5 Community Staff Nurse to join the multidisciplinary Care Community Team in Neston. This part-time position (29 hours per week) involves providing high-quality nursing services and holistic assessments to patients primarily in their own homes, promoting a person-centered approach to care. Successful candidates will collaborate with various healthcare professionals to ensure seamless care for patients, aiming to prevent unnecessary hospital admissions.
