The role of Primary & Social Care Coordinator with Imago Community in London involves supporting Unpaid Carers throughout Lewisham by collaborating with various stakeholders, including Primary Care Networks, GP practices, local authorities, and community organisations. The Coordinator will focus on identifying and engaging with Unpaid Adult Carers, Young Adult Carers, and Young Carers, while enhancing services related to registration, communication, support, and accessibility. Responsibilities include developing relationships, establishing action plans, and promoting 'Carer Friendly' environments to foster the wellbeing of these individuals.

Ideal candidates should possess personal or professional experience working with Unpaid Carers or vulnerable groups, and have a practical understanding of the challenges they face.
