The role of "Hospital Discharge Placement & Review Officer" involves collaborating with hospital discharge staff, care homes, individuals, families, and carers to facilitate effective discharge planning and care coordination within Renfrewshire. This position, situated primarily at Council Headquarters but also requiring work at the RAH, focuses on timely negotiation and planning to combat delayed discharges while supporting older adults in various environments.

Candidates must possess experience in managing complex service demands, conducting assessments, and effectively communicating with diverse stakeholders. Strong interpersonal skills, the ability to prioritize and multitask, and proficiency in IT and data management are crucial.
