As part of a multidisciplinary team the post holder will carry out personal care duties for patients in support of the registered nurse and other relevant professional practitioners where appropriate. In partnership with the registered nurse assess and review the plan of care using the Activities of Daily Living model for patient care. Implement the plan of care agreed with the registered nurse
Undertake associated clerical and patients centred duties as delegated by the registered nurse.
