The Hospital Transition Team Paraprofessional at Therapeutic Alternatives Inc. in Aberdeen is responsible for providing short-term support to individuals transitioning from psychiatric hospitals or residential substance abuse treatment programs. This role involves crisis response, implementing individual habilitation plans, and assisting consumers with daily living skills, socialisation, and transportation to community services.
