The Senior Community Navigator role at Berkshire Healthcare Foundation Trust is a part-time position (18.75 hours per week) aimed at enhancing the wellbeing of individuals over the age of eighteen in the Wokingham community, particularly those feeling isolated or using mental health services. The role involves facilitating social connections, conducting one-on-one Wellbeing Mapping sessions, and organizing social network meetings to promote community engagement. Successful candidates will work collaboratively with various BHFT teams and community organisations while focusing on person-centered recovery and the integration of services to reduce social isolation.

Candidates should have a strong understanding of the Let's Connect Wellbeing Network and a commitment to non-clinical, integrated care approaches. The position requires flexibility, self-organisation, and the ability to travel widely across Berkshire.
