Ranworth PCN is looking for an experienced Practice Nurse for a full-time, permanent role at their Green Elms Surgery in Clacton-on-Sea. The successful candidate will play a key role in delivering high-quality healthcare services to a diverse patient population, focusing on various clinical areas including infection control, chronic disease management, and health promotion. The nurse will also support the creation of the Infection Prevention and Control annual statement, develop care programs for frail and housebound patients, manage acute and chronic conditions, and assist GPs with minor surgeries.
In this senior position within the multidisciplinary team, the Practice Nurse will assess patient needs, create and implement individual treatment plans, and support patients at risk of developing long-term health issues. Ranworth PCN are seeking an experienced practice nurse to work at their Green Elms Surgery on a full-time, permanent basis to support the provision of high-quality healthcare and diverse patient list. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population, responsible for a number of clinical areas including infection control, chronic disease management, well women and well man clinics and health promotion.
- Support the production of the IPC annual statement in conjunction with the nurse manager.
- Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
- Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
- Develop, implement and embed health promotion and wellbeing programmes.
- Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.
- Diagnose and manage acute and chronic conditions.
- Review patient medications to enhance compliance.
- Assist GPs with minor surgery.
- Support the clinical team.
