MENTAL HEALTH MULTI-DISCIPLINARY NHS
The role of Primary Care Mental Health Practitioner at Epsom PCN within Surrey and Borders Partnership NHS Foundation Trust is a full-time position focused on enhancing integrated mental health services within local GP practices. The practitioner will collaborate with a multi-disciplinary team including Clinical Leads, Community Connectors, and specialists to provide assessments, brief interventions, and care navigation for adults with significant mental health challenges. Responsibilities also include managing clinical referral priorities and supervising the Community Connector role.

The practitioner will work across multiple GP sites in Epsom, addressing the mental health needs of individuals who frequently seek support from their general practitioners. The role emphasises developing effective links between primary and secondary care mental health services, promoting a transformative approach to community mental health care. Flexibility to travel between various locations is required for this position.
