The successful candidate will work across various schemes such as the Good Neighbour Scheme, Hospital to Home, A&E to Home, and Back Home Scheme, offering practical assistance, emotional support, and helping clients regain confidence in independent living.

The role entails collaborating with health and social care professionals to facilitate safe discharges, delivering low-level interventions to prevent deterioration in health, and ensuring that individuals are reintegrated into their communities. Responsibilities will include managing referrals, conducting home visits for assessments, providing guidance on community resources, and supporting informal caregivers.
