West Sussex County Council is seeking an enthusiastic and committed Social Worker to join their Hospital Social Work Teams in Chichester. The role focuses on supporting the adult population through the Discharge to Assess model, ensuring timely assessments for individuals recently discharged from hospital and facilitating their long-term care needs to promote independence. As a Senior Social Worker, the successful candidate will be responsible for providing a statutory social work service, conducting strength-based assessments, planning and reviews, and safeguarding inquiries while collaborating with health professionals and stakeholders.

The ideal candidate will possess experience in managing complex cases and will be adept at navigating competing priorities in a dynamic environment. They must stay compliant with relevant legislation and organisational policies, while promoting inclusivity and equality. The role also includes participating in a duty function on a rota basis and requires robust communication skills to engage effectively with individuals and the public.
