The Specialist Nurse Assessor for Kingston & Richmond at South West London Integrated Care Board will be responsible for evaluating clients with Learning Disabilities across various settings to determine their eligibility for Fully Funded NHS Continuing Healthcare (CHC) or Funded Nursing Care (FNC). In this role, the post holder will coordinate a multidisciplinary team, ensuring timely assessments and adherence to national performance targets while supporting clients and their families in identifying suitable care packages. The position involves managing a caseload, conducting assessments, and providing expert advice on Learning Disabilities to contribute to high-quality care.

Additionally, the specialist will support the Continuing Healthcare team by maintaining effective communication networks, prioritizing workload, and managing complex cases. The post holder will also engage in continuous development efforts, including training and recruitment, while ensuring compliance with established guidelines and protocols. Responsibilities further include handling enquiries related to CHC and FNC, participating in review meetings, and ensuring that all activities align with professional standards and data protection regulations.
