The Advanced Clinical Practitioner at The Grange Medical Practice in Huddersfield will be responsible for managing approximately 30 patient contacts daily through various consultation methods, including face-to-face, telephone, and video appointments. The role requires an independent prescriber who is well-versed in current prescribing guidance and NICE guidelines, capable of assessing, diagnosing, and managing a diverse range of acute and ongoing health concerns within a multi-disciplinary team.

Key responsibilities include conducting independent patient assessments, making referrals to secondary care, reviewing and managing pathology, and providing home visits and ward rounds at care and nursing homes as needed. The position demands a commitment to quality patient care while accommodating the specific needs of the patients and the practice, aligned with NHS contractual requirements.
