Mersey Care NHS Trust is seeking Advanced Clinical Practitioners (ACPs) for the role of Community Matron across various sites in the Sefton area. This pivotal position involves proactive caseload management and first-contact care for patients with acute long-term conditions, delivered in their homes and registered care facilities.
Key Responsibilities:
  • Deliver clinical leadership as part of the Integrated Community teams.
  • Manage and coordinate care for patients with complex needs autonomously.
  • Assess and plan comprehensive care programs to enhance patient independence and health outcomes, in collaboration with multidisciplinary teams.
  • Utilize advanced clinical skills to monitor health conditions and facilitate interventions based on best practices.
  • Maintain effective communication with patients and healthcare professionals, ensuring patient-centered care.
  • Legally and professionally manage a diverse caseload while ensuring adherence to established guidelines and policies.
  • Participate in professional development and contribute to team training and knowledge sharing.

Qualifications:
  • Candidates must hold an MSc degree in Advanced Clinical Practice and demonstrate relevant clinical experience.
