The Herefordshire and Worcestershire Health and Care NHS Trust is seeking a dynamic and enthusiastic Senior Crisis and Home Treatment Clinician to join their Crisis and Home Treatment Team, following an internal promotion. This role involves providing crisis assessment and home treatment to individuals experiencing acute mental health crises in a least restrictive environment, while also facilitating early planned discharges from inpatient services. The position operates a 24-hour service, requiring the clinician to work 37.5 hours a week across various shifts, including nights. Responsibilities include conducting comprehensive risk assessments, engaging with patients and families, and collaborating with multidisciplinary teams to enhance recovery and crisis management.

The clinician will work autonomously and take on responsibilities for quality of care provided to patients aged 17 and over. Engagement in education for partnership agencies and students, along with thorough documentation in line with Trust policies, is also expected. The role calls for a proactive approach in liaising with various stakeholders, including GPs and social care teams, to ensure a holistic support system for clients in crisis.
