The Midlands Partnership NHS Foundation Trust is seeking a part-time Community Staff Nurse to join their Stafford Town PCN team. The role involves delivering high-quality nursing care in a community setting, collaborating with GPs and multidisciplinary teams to address various healthcare needs and prevent avoidable hospital admissions. Candidates must be a qualified nursing practitioner with current NMC registration, although previous community experience is not mandatory. The position requires flexibility in working hours across a seven-day week and encompasses responsibilities such as assessing care needs, developing and implementing care plans, and promoting health and wellbeing.

The Community Staff Nurse will play a vital role in evaluating and managing diverse health conditions, focusing on improving patient independence and preventing health inequalities. Duties include conducting assessments, formulating care plans for various needs, and providing palliative care. The role emphasises a commitment to personalized, high-quality care, with opportunities to work closely with various healthcare teams to enhance patient outcomes within the community.

We have an exciting opportunity for a Community Staff Nurse to join our Stafford Town PCN team on a part-time basis. We are an innovative service covering GP practices in the Stafford locality and we are seeking dedicated, compassionate and highly motivated nurses.

As a Community Staff Nurse you will deliver a high quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions.

You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care, adhering to policies and procedures.

You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.

You will be required to work flexibly on a rota system across a 7 day working week.

You will be part of the District Nursing service providing healthcare and will work in a variety of settings.

This is a dynamic post that requires the post holder to work closely with uni/multidisciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.

Duties to include:

To assess new service user referrals and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. The care/support plans may include the following:

Promoting independence/self management and maintaining safety Increased functional ability

Improvement in health and wellbeing including mental health

Interventions in disease/condition management

Prevention and reducing of health inequalities

Admission avoidance

Early supported discharge

Case management/ treatment/ care plans

Palliative and end of life care

Undertake Tissue Vaibility Risk Assessments

Form a professional judgement of an individual's health condition

Prioritise treatment and care for individuals according to their health status and needs

Bladder, catheter and bowel care
