The Mental Health Community Support Worker position with Herefordshire and Worcestershire Health and Care NHS Trust involves joining the Neighbourhood Mental Health Team (NMHT) in North Herefordshire to provide accessible mental health support within the community. The role includes implementing care plans, managing a caseload, and working collaboratively with a multi-disciplinary team composed of various healthcare professionals. The worker will support individuals in their recovery journey, facilitate access to appropriate services, and provide both short-term and longer-term interventions based on clinical needs.

Candidates must demonstrate flexibility, organisational skills, and preferably have experience with graded exposure work.
