£35,392 - £42,618 per annum (pro rata for part time hours)

We are pleased to offer this new and exciting opportunity to join the new Mid and South , Older Adult Admission Avoidance & Home Treatment Team. As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs. Fixed term contract contract covering 37.50 hours per week commencing January 2025

About us

As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support. You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex
