Southend West Central Primary Care Network has a fantastic opportunity for an enthusiastic and committed nurse practitioner to join our PCN on a full-time basis, for 37.5 hours per week. There is flexibility to discuss hours at interview.
The five practices within our PCN include Prince Avenue Surgery, The Valkyrie Surgery, Southend Medical Centre, Dr Bekas Medical Centre and Scott Park Surgery.
The main aim of this role is to increase health and wellbeing of patients through improved screening, identification, and disease management. This will entail a mixture of ward rounds, outreach events home visits and some clinics as directed.
- Clinically examine and assess patient needs from a physiological and psychological perspective & plan clinical care.
- Assess diagnosis, plan, implement & evaluate interventions/treatments for patients with complex needs.
- Proactively identify, diagnose & manage treatment plans for patients with, or at risk of developing a long-term condition.
- Diagnose & manage both acute & chronic conditions, integrating both treatment methods into a management plan.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national & practice protocols & within scope of practice.
- Implement & participate in vaccination & immunisation programmes for both adults & children.
