The Northern Lincolnshire and Goole NHS Foundation Trust is seeking an experienced Therapy Assistant or existing Assistant Practitioner to join their Community Therapy Team in Grimsby. This role involves working with patients in their homes and at various care facilities, assessing their needs to provide suitable interventions aimed at enhancing their independent functioning. Responsibilities include conducting assessments for assistive equipment, implementing rehabilitation plans, and offering advice on home adaptations, all while collaborating with a multidisciplinary team under the supervision of registered Occupational Therapists and Physiotherapists.

Candidates should possess a Foundation Degree in Professional Practice or an equivalent Level 5 qualification in a relevant area.
