CASELOAD MULTI-DISCIPLINARY
The "Learning Disabilities Screening Nurse" position in Plymouth with Livewell Southwest is aimed at enhancing health outcomes for individuals with learning disabilities by increasing their participation in five key national screening programs. This role involves collaborating with various stakeholders, including healthcare providers, community staff, and advocacy organisations, to ensure that individuals with learning disabilities receive timely access to essential screenings that can lead to early identification and management of health issues. The nurse will facilitate engagement with screening initiatives, provide educational support, and advocate for adaptations to make these services more accessible.

In this multifaceted role, the nurse will work as part of a Multi-Disciplinary Team (MDT) to develop training resources, assess and manage health risks, and build relationships with local health services. Responsibilities include reviewing GP learning disability registers, planning targeted interventions, and raising awareness about the importance of screenings, while ensuring sensitive communication with patients and their families. The position emphasises health equity, aiming to directly address barriers to screening uptake and working toward better long-term health for the learning disabilities population in Plymouth, with potential expansion across Devon in the future.
