The Ward Tracker role at Stockport NHS Foundation Trust focuses on facilitating timely discharges for patients who no longer require acute care. The post holder will work closely with the Hospital Discharge Team and the Discharge to Assess Hub, ensuring effective communication between these teams and the wards. Key responsibilities include attending ward rounds to manage discharge plans, maintaining accurate discharge checklists, and transferring patients to the discharge lounge as needed.

Additionally, the role involves updating ward information systems and ensuring patients and their carers are informed about discharge requirements. The successful applicant must share the Trust’s commitment to safeguarding and will need a Standard DBS check, with costs deducted from the first salary.
