The Limes Surgery in Hoddesdon is seeking a full-time Lead Practice Nurse to oversee and guide a dedicated nursing team. The ideal candidate will be proactive and experienced, responsible for delivering safe, effective, and patient-centered care to the practice population. Key responsibilities include managing care for patients with long-term conditions, acting as the clinical lead for the nursing team, implementing practice standards, and ensuring the achievement of performance targets related to quality of care and local initiatives.

This role entails leading nursing team meetings, representing nursing interests, and overseeing the Quality and Outcomes Framework (QoF) strategy and other enhanced services, such as immunisation and flu campaigns.

- Managing care for patients with long-term conditions.

- Overseeing the QoF strategy (and other Locally Enhanced Services) for LTC management and ensure key performance targets are met (immunisations, flu campaigns, etc.).
