The Band 4 Rehabilitation Practitioner in Vocational Rehabilitation at Northampton General Hospital will work within the Northamptonshire Community Stroke Team (N-CST), providing support and rehabilitation to stroke patients in their homes and community settings. The role involves managing a caseload of non-complex patients, executing treatment plans under the guidance of qualified therapists, and assisting in the implementation of vocational rehabilitation initiatives. Responsibilities include conducting assessments, developing treatment programs, and advocating for patients with employers and occupational health providers, thereby promoting the patients' functional independence and return to work goals.

The position requires the post holder to work independently, undertake home and workplace visits, and engage in data collection for service development and auditing. The practitioner will collaborate closely with a multidisciplinary team, ensuring the delivery of integrated care while respecting the diverse needs of patients and their families. Additionally, the role involves participating in staff training, maintaining accurate records, and adhering to national and organisational guidelines, along with contributing to service improvements and addressing the needs of the stroke population across Northamptonshire.
