The Senior Clinical Pharmacist role at Greenwich Health Ltd involves working within the Valentine Primary Care Network in London, where the post holder will be part of a multidisciplinary team focused on improving patient care for those with chronic conditions and co-morbidities. The role is patient-facing, requiring proactive management of medication-related queries, support for repeat prescriptions, acute prescription requests, and medicines reconciliation during hospital discharges. The pharmacist will provide clinical leadership on medicines optimisation, participate in quality improvement initiatives, and be a resource for general practice staff regarding medication issues.

Candidates should possess experience in primary care and preferably hold independent prescribing credentials. Responsibilities include conducting clinical medication reviews, managing long-term conditions clinics, providing telephone and in-person support for patients, and monitoring patients post-discharge. Effective communication with various healthcare professionals and adherence to safety and quality standards are essential. The position requires a commitment to continuous professional development and involvement in the PCN’s initiatives to enhance healthcare delivery and patient outcomes.
