The Clinical Practitioner role at Great Western Hospitals NHS Foundation Trust in Swindon involves delivering effective nursing care to patients in their homes and community settings as part of the Urgent Community Response and NHS@Home teams. The service operates daily from 8 AM to 10 PM, requiring a commitment to high-quality, evidence-based nursing care within a community setting.
