Band 2  PERMANENT  GOOD SALARY 

To assist with general wellbeing, creating a supportive and re- enabling environment promoting maximum independence.

Our work in Primary Care Networks (PCN) is at the very heart of Stockport PCN Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time".We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers.We are looking for enthusiastic individuals who would work with the Community Nurses and Adults Social care staff to provide a first-class service to the residents of Stockport. Line management will be provided by Stockport foundation trust, under NHS terms and conditions. On appointment successful candidates will be expected to complete the Care Certificate.

To work within a multidisciplinary team delivering social and nursing care (appropriate to the post) to clients in their own home. Care delegated by an appropriate practitioner utilising personalised care plans as appropriate.

Providing patients with support in administering medications, eye drops, wound care and supporting your colleagues to provide excellent care in the patients' home.

Working 15 hrs/week between 18:00pm-23:00pm.

The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and Palliative patients symptom control.

If candidates who are appointed do not hold the Care Certificate, an opportunity to complete will be available. The post holder
MUST
hold a driving licence with access to a car to fulfil this role.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively, and sharing information appropriately.

If successful, this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary.

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
