Sheffield Health and Social Care NHS Foundation Trust is seeking a Band 5 Care Co-Ordinator to join their Early Intervention Team, which focuses on supporting individuals experiencing their first episode of psychosis. The role involves conducting comprehensive assessments of clients' health and social care needs, managing clinical risks, formulating and reviewing care plans, and delivering evidence-based interventions. Collaboration with service users, carers, and partner agencies is key, as is maintaining accurate care records and developing strong relationships with primary care colleagues.

In addition to clinical responsibilities, the Care Co-Ordinator will review their caseload, provide supervision to junior staff, and participate in their own professional development, including mentoring learners and engaging in relevant training. The position requires a multidisciplinary approach and the ability to work sensitively across diverse cultural backgrounds.
