The Health Visitor role at HCRG Care Group in Salisbury involves delivering a high-quality, proactive service for children, particularly focusing on the 0-5 Healthy Child Programme. In addition to providing care and support to children and families, the Health Visitor will take on a leadership role within the team, offering supervision to colleagues and community nursery nurses, while also contributing to multi-agency assessments. The aim of the service is to reduce health inequalities, safeguard children and improve outcomes.

We currently have a full time opportunity working 37.5 hours across the weekbased at the Highpost Hub, Salisbury.

As a Health Visitor within HCRG Care Group, you'll join a team who are focused on making an impact on the development and wellbeing of children in our local community.

You will:

Lead and deliver the Healthy Child Programme within a designated population area
assess health needs and implementing interventions with a focus on personalised or targeted responses, utilizing team skills effectively.
Take accountability for children's outcomes collaborating with partners to address public health needs
Promote health and well-being through the Healthy Child Programme and referrals to partner agencies as needed
Contribute to multi-agency early support assessments for children and families, taking on a lead professional role when necessary.
Manage a caseload autonomously ensuring effective assessment, intervention, and management.
Model excellence in Specialist Community Public Health Nursing practice, providing supervision to learners and colleagues.
Provide management and leadership to community nursery nurses and other team members, including caseload allocation and monitoring.
