Take accountability for children's outcomes collaborating with partners to address public health needs.

As a Health Visitor youll join a team who are focused on clinical excellence! Your days will be rewarding and variedand include delivering a high quality, proactive service for children and the 0-5 Healthy Child Programme. The aim of the service is to reduce health inequalities, safeguard children and improve outcomes.

We currently have a full time opportunity working 37.5 hours across the weekbased at the Highpost Hub, Salisbury.

As a Health Visitor within HCRG Care Group, you'll join a team who are focused on making an impact on the development and wellbeing of children in our local community.

You will:

  • Lead and deliver the Healthy Child Programme within a designated population area
  • assess health needs and implementing interventions with a focus on personalised or targeted responses, utilizing team skills effectively.
  • Take accountability for children's outcomes collaborating with partners to address public health needs
  • Promote health and well-being through the Healthy Child Programme and referrals to partner agencies as needed
  • Contribute to multi-agency early support assessments for children and families, taking on a lead professional role when necessary.
  • Manage a caseload autonomously ensuring effective assessment, intervention, and management.
  • Model excellence in Specialist Community Public Health Nursing practice, providing supervision to learners and colleagues.
  • Provide management and leadership to community nursery nurses and other team members, including caseload allocation and monitoring.
