The CAMHS Transitions Coordinator position at South West London and St Georges Mental Health NHS Trust in Teddington is a full-time, 12-month fixed-term role focused on supporting young individuals aged 17 as they transition from Child and Adolescent Mental Health Services (CAMHS) to adult mental health services. The coordinator will work collaboratively with families, multiple agencies, and teams to ensure that young people are adequately prepared for this transition, thereby promoting their mental health and independence. Responsibilities include leading safeguarding initiatives, conducting assessments, managing complex case meetings, providing group interventions, and contributing to the development and implementation of transition protocols and training for staff.

The ideal candidate will possess strong leadership and communication skills, demonstrate a track record of systemic collaboration, and be adept at engaging with young people and their families. In addition to direct service delivery, the role encompasses service development, teaching, supervision, and the continuous evaluation of practice to address the diverse needs of young people from various backgrounds. The position is designed to enhance mental health outcomes by ensuring a seamless transition for young clients into adult services, ultimately reducing the risks associated with this critical period of change.
