Band 4: £31,944 to £34,937 a year Per Annum including inner london HCAS  FIXED TERM

The Patient Pathway Co-ordinator roles is to provide efficient and high quality secretarial and administrative support to the clinical and management teams in the day to day running of the sudden cardiac death and sudden cardiac arrest pathways working in close co-operation with the Inherited Cardiac Conditions (ICC) coordinator, ICC consultants, clinical genetics staff, Intensive Care Unit staff, pathologists and coroner's officers across South West London, Surrey and West Sussex. This important programme is funded by NHS England through the Genomic Medicine Service Alliances and the British Heart Foundation.

The Patient Pathway Coordinator will report directly to the Assistant Service Manager for Cardiology. They will be expected to work autonomously but under direction. The role is key to delivering an effective service to families bereaved through sudden cardiac death (SCD) and survivors of cardiac arrest (SCA). The postholder will need to liaise closely with both the ICC staff and the clinical genetics team to ensure seamless care and support for these individuals and their families.
