Battersea Primary Care Network in London is seeking a dedicated Clinical Pharmacist to join their expanding pharmacy team, which consists of a lead pharmacist, deputy lead pharmacist, and several junior and senior clinical pharmacists. The role involves patient-facing responsibilities focused on chronic disease management, medication reviews, and ensuring safe and quality prescribing practices. The successful candidate will possess strong clinical, management, and communication skills, be capable of working flexibly across practices, and contribute to the development of pharmacist-led pathways for various health conditions.

Key responsibilities include conducting chronic disease reviews, providing expert medicines advice to practice teams, improving prescribing practices, and supporting the implementation of quality initiatives. Additionally, the Clinical Pharmacist will engage in medication reconciliation, liaise with community and hospital pharmacy services, and assist in public health campaigns. This position offers a unique opportunity to develop and refine pharmacy services within a supportive network, with a strong emphasis on education and training.
