We are looking to recruit a highly motivated, caring, compassionate and experienced nurse to a Band 6 Sister/Charge Nurse post in the Sunderland Out of Hours Palliative Care Team. The team are part of an Integrated Specialist Palliative Care Team, and provide high quality palliative and end of life care in the out of hours period which is timely and meets the needs of patients, carers and familys. This relies on a team of knowledgeable and skilled practitioners who can work effectively with the multi-professional teams across different care environments. You will be a key leader of the team providing specialist palliative care, advice and support to patients, carers and professionals. The main aims of the service are to:
You will be expected to maintain agreed standards of care, directly protecting and promoting patients dignity, choices, self esteem and well being at all times. You will be working within an out of hours service that operates between the hours of 16:00 08:45 Monday to Friday and 24/7 on weekends and bank holidays. There are currently three shift patterns in operation to cover the hours of the service. Shift patterns are reviewed regularly and may be subject to periodic change.
The nominal base for this role will be St Benedicts Hospice and Centre for Specialist Palliative Care in Sunderland, however you may be required to work across the whole of Sunderland and South Tyneside.
